Full-Time RN Care Managers We are seeking RNs to join our Family Care team. These RNs will be responsible for assessing, planning, intervening/delivering, coordinating, monitoring and evaluating services for frail elderly, and adults with physical and developmental disabilities. They will also participate in delegation and oversight of care to home health agencies, and participate in discharge planning. Requirements include: Associate degree in Nursing, State of Wisconsin licensure. Ideal candidates will have experience in a community based or long-term care settings.

Full-Time Care Managers We are looking for a Human Service professional to fill our Care Manager position. This individual will use interview, observation and member input to assess physical/health status, environment/ safety, independent living skills, and functional ability related to member goals/ outcomes. Another key responsibility to this position is to Facilitate the development of the Individual Service Plan (ISP)/Member Centered Plan (MCP) and ensure the ISP/MCP reflects the member's goals/outcomes to maximize quality of life for that individual. Requirements include: Bachelors degree in Social Work or a related field and a valid State of Wisconsin drivers license.
