STAFF PHARMACIST--SFT7, 1ST SHIFT

The pharmacist performs a combination of clinical and distributive pharmacy services necessary for the provision of pharmaceutical care to patients seen at the Froedtert & the Medical College of Wisconsin Clinical Cancer Center. To accomplish this objective, the pharmacist shall interact with other members of the health care team to proactively to address drug therapy issues. In conjunction with Froedtert's mission focusing on customer service, confidentiality, education, and world class excellence; the pharmacist assures that patient care is maintained through compliance with the department's quality improvement plan and the JCAHO standards for pharmaceutical services. The ambulatory hematology-oncology staff pharmacist provides comprehensive pharmaceutical care to patients through integrated drug distribution, clinical services and serving as a source for drug information.

The individual in this position will report directly to a Pharmacy Manager and will have responsibility for no direct reports.

Professional Requirements:

1. B.S., M.S., or Pharm. D. degree received from an ACPE certified school of pharmacy.
2. Licensed as a pharmacist in the state of Wisconsin (or eligible)
3. Minimum of one year hospital or three years of other pharmacy experience. (preferred)
4. Successful completion of ASHP Pharmacy Practice or specialty residency (preferred)
5. Active membership in the Pharmacy Society of Wisconsin (PSW) (preferred)
6. Active membership in American Society of Health-System Pharmacists (ASHP) and/or other national Pharmacy organizations, as approved by Pharmacy Administration. (preferred)
7. Demonstrates effective use of computers and applications (e.g. MS-Word, Excel, Outlook) as appropriate to practice and communicate effectively.
8. Maintains competency in unique aspects of drug therapy for hematology-oncology patients, including factors related to age, organ function and disease states.
