Continuing Care Nurse Coordinator / HHH20918

Position Description:Facilitates the development and completion of referrals from System/ sources for the Continuing Care Services Division (CCSD). Contacts payer sources to ensure coverage of home care and transitional/extended care services when necessary. Disseminates referral information to the appropriate offices/facilities. Provides education on System services and provides information on community resources to meet callers' needs. Position is available at St. Mary's Medical Center - Racine.THIS POSITION QUALIFIES FOR A JOB SHARE OPPORTUNITY.Qualifications:Completion of a Registered Nurse Program. Previous home care, transitional/extended care, or discharge planning experience required. Familiarity with Wheaton Franciscan Home Health & Hospice and facility patient scheduling and procedures preferred.
