Community Case Manager
Job Description Provides expert patient care for general and defined managed care populations. Collaborates with physicians, other health care providers including managed care organizations to assess, identify outcomes, plan, sequence, and evaluate client services across care continuum. Focuses on prevention, maintenance, restoration and education.

Identifying outcomes derived from assessment data and in cooperation with family and others involved in client's care.
Gathering comprehensive data through client interview and situation evaluation.
Making referrals based on client assessment, needs and input.
Utilizing research to discover, examine and evaluate knowledge, theories and creative approaches to practice.
Working with health care providers and organizations to develop, provide and evaluate outcomes for needed care based on assessments of client population groups.
Creating Interdisciplinary care relationships that are collaborative, proactive and client focused to facilitate and maximize client outcomes.
General Qualifications
Master's Degree in nursing with 3 years of experience in advanced practice or equivalent role.
Wisconsin Registered Nursing License.
3-5 years previous work experience in Case Management/Coordination or in equivalent practice arena
Excellent oral and written skills
Experience working with computer programs, including Microsoft Word and Excel
Desired Qualifications
Experience in nursing care management with experience in a leadership role preferred
