Prefer experience in Obstetrics and/or Pediatrics. Duties will be to manage CHC clients with special, catastrophic and medically complicated needs through comprehensive evaluation and through coordination of CHC resources. The Case Manager/Disease Manager will participate in discharge planning of these members and will participate in annual quality improvement reviews of participating physicians’ offices.
